hello there.

i use the college consultation forms from my college reflexology at the moment but i wanted to make up my own when i start treatimg clients myself, as i think the college one is quite....well.....usless and long winded.

so i was wondering, does anyone have any suggestions of what i should put on my consultation form, and what i shouldnt leave off?

also can you use the same consultation forms for all treatments? well for mine it will be reflex and IHM?

Hi Sarah, do you have Louise Tucker's An Introductory Guide To Reflexology? She has a great form in there.

RE: Consultation forms

Hi Sarah

I do Aromatherapy and found a good form online. Don't know how much would be the same but it might be worth taking a look at. The site was aromacaring.co.uk.
