My Sister (married, aged 43 with two children) has suffered for years with painful Endometriosis. About six months ago she had a Hysterectomy. But they left one Ovary inside her so that she wouldn't go into an immediate menopause. The Surgeon tried to remove as much scar tissue inside her as he could.
Now my Sister has suddenly started to suffer with a painful tummy again, after months of being painfree and looking the best she has been for years.
I have heard that still having an Ovary can produce the hormones to create Endometriosis. Also, that whatever Scar tissue is removed, even if a few cells are left behind, it can still cause Endometriosis.
She had Xrays and Scans and there is no obvious problem, though only a laparoscopy would find the Scar tissue.
It's so frustrating and painful for her.

I used to be a pharmacist at a local Womens' hospital
Endometrial plaques can exist outside the womb - so it can't be ruled out - but it is unlikely to be that given the sudden onset of pain you describe. They have ruled out any treatable causes of the pain. If it is still endometrial plaques I think their options to treat are limited. OK they could remove the remaining ovary BUT the risks of doing that probably outweigh the benefits....unless the pain does not respond to pain killers.
She really needs to talk to her GP or gynaecologist.
