taster sessions and consultation forms

when doing taster sessions what do you put in a consultation form as you do not have long to do this, do you just get them to sign and say they have none of the contraindications?
do you just do that with there name and the date??
any help would be great

i don't usually have a consultation form for taster sessions (not enough time). I usually have a list of contraindications which I get them to read through and if any apply we discuss and see if treatment is appropriate or not.

I take their name and address and ask them if they have any medical conditions then get them to sign a disclaimer - takes a minute or two max. I like the idea of a list of contra-indications for them to read through Binah.

I always do a quick consultation with a list of contra-indications.
While helping out a friend with a pamper party a few weeks ago I nearly gave I.H to a client with epilepsy as she didn't have consultation sheets... luckily the client's friend asked if it was ok to treat her just before I started.

Very close shave for me.

taster consultation forms

I complete a short consultation form for tasters, it contains the contraindications and the contact details especially an email address so I can inform them of offers and events to come

I have a piece of A4 with a list of conditions that prevent me from treating and a list that the client must inform me about. Below they sign to say they've read and understood, so basically everyone signs the same form.
