Hi
I am a few weeks into my Indian Head course and really enjoying or was until we were told that we needed to write up 4 case studies 2 of which need to be in next week. I have almost finished my first two people but haven't a clue how to write it up. The tutor has given us a sheet with the information we need to include. I don't really know how to set it out so if anyone has any advice I would be really grateful. I have a copy of the Helen McGuiness book but the case studies in there cover the treatments in one go and we have to do each one on a seperate page. Help

I am also doing IHM - we did our final exam last night but still do practical till March. For VTCT we have to do 3 case studies on 3 people!
This is the info we write down:
Date
Client Reference No (Because of confidentiality we do not put their name on this sheet - their details are listed elsewhere)
Date of Birth
Occupation
GP Name, address and Tel no.
Medication
Medical History
Additional information (Things like was the client nervous, somebody elderly, somebody bold etc.)
Client objectives/Expectations (i.e migrane sufferer, relaxation, etc.)
Treatment plan (after speaking to the client you will decide on the treatment -i.e if the client has acne you will note to work around that, or if you are not doing a full massage you will note that)
Treatment carried out (the date and usually I just write see above for the treatment given)
Aftercare/Homecasre advise (Whatever you told the client (No hot treatments for 24 hours, no alcohol etc)
Client Feedback (Complete this after the treatment - if a client says realy liked the bit on the face or hated that bit in the neck you can make note of this and in the next treatment do more on the face and less on the neck - this will thus go into your next treatment plan for this patient)
Client signature (you get this before you do the treatment but after you completed the form.

Hi Everyone
Thank you for your advice. I had to hand in two completed case studies before Christmas. We were given them back last night and I passed them both - really pleased only two morehome studies and a college one to go.
