I am looking for some advice. I am sure you will be able to help. Do you get clients to fill out consultaion forms? If you do what information do you ask for on these?
I am trying to make sure I cover all bases when delivering Reiki to clients. Should I bee keeping these for my own records too?

Did your Reiki teacher not cover this aspect with you? I include a sample intial consult form & a treatment record form for students to adapt for their case studies. If you are looking for ideas you could look on [url]William Rand's website[/url] and download and adapt his sample treatment sheet.
Breifly - You need to record information so that you can get a picture of the client & tailor the treatment to that person's needs, record any feedback, changes etc. and you must be mindful of Data Protection and keep your records in a safe place under lock & key.
I have found it useful to get 3 kinds of info at the initial session.
1. Some basic info such as name, address, email & telephone number which are very useful if you need to get hold of someone.
2.Basic medical history and lifestyle info such as childhood & adult illness, current conditions or illnesses and medication. + smoking, drinking and exercise. Why have they come to see me and are there lifestyle issues that need addressing? Are there any particular physical issues?
3. Some attitudinal and emotional questions designed to elicit the client's take on their life and how they deal with relationships, their work and their emotional flow. eg. are they always unhappy or angry? Happy in relationships or job?
I also dowse their chakras and record what I find as well as any responses or extra hand positions used.
In addition you need to keep session-by-session treatment records so that you can record progress, changes etc. It is useful to be observant and listen for what the patient "doesn't" tell you in the initial consult but what comes out in casual conversation during & after the session that may be very pertinent. It is not uncommon for someone to remember an illness, accident, emotional upset or something that they didn't mention during the initial consult for example.
