please could I ask what your consultation forms look like? What info do you include and what do you ask for?

I'm a little confused by your question as I would have thought this would have been covered in your training so I'm not sure whether I'm missing what you really want to know. However, in a nutshell, I ask clients details such as:
  • Name, dob, address and occupation
  • Why they've come for treatment
  • Medical history
  • Lifestyle information
Is this what you were asking?

Thank you for your reply. Sorry to confuse you. It was of course covered in my course but not in great detail and it was quite a few years ago now. It was only recently that I was looking at my form and wondering if i really need all the info I ask for or if there was other info I am missing.

For example, one of the questions is marital status. Do I really need to know if they are married or not?? Maybe a better question would be something along the lines of what kind of lifestyle they have like you have mentioned?

If anyone has any more tips on this I would love to hear them.

Hiya,

Have you joined a professional body yet?

I am with the AOR and they actually supply approved consultation forms and follow up forms which cover all the necessary information that you need to obtain from your client.

I was trained in gaining alot more info from the client than the essentials that are covered on the form. This is really good to gain a complete oversight of your client and look at their whole lifestyle however it doesn't really work in practise as you don't have too much time with them.

If a client books up for a whole course of treatments then you can add your own question list to the pre printed ones from the association but at least you know you have covered the basics.

Hi I got mine on e-bay they came as PDF file and they are great, I dyslexic so it save me a lot of time trying to type all he information myself ,

The consultation forms your given when training are only a guide line. I frequently change and up-date mine.

Although it is necessary to get an insight in to why a client comes to us and a little background, I think it depends on the therapy you are giving as to what depth you need to go into things. When doing my own research years ago, I felt quite irritated by some of the questions, even marital status, what the heck, why does that matter. I don't think we should pry too much. I have my own forms but when joining the FHT recently I notice you can purchase them there.

That's really interesting Jeannie. It never occurred to me that it may irritate or insult a client to ask about their marital status. You'd probably hate to have a consultation with me! I always ask whether they're married, living with anyone or in a relationship, I really want a very clear overview of the client, I find that having a 'birds eye' view of the clients (perspective on) life - in general and day to day is really helpful when treating them holistically. The more details the better! I find that's when I'll start to see patterns or correlations between any health issues and their emotional/ mental state of mind, and their life style choices. The people closest to us, especially those we live with, I believe have a large impact on our happiness our feelings of security in the world, or equally our frustration or anger, and I also believe that our consistent thoughts and emotions our responsible for most (all...) of our physical issues. It's all necessary information in my eyes. I take a full systems overview which means asking about pooing frequency and consistency (constipated etc), and with the ladies their periods- blood flow, colour pmt symptoms etc. I think asking if they're married is the least of their worries!

So I ask a lot of questions, I'll often take up to an hour, this is how i was trained (AOR) I do think it helps, well myself anyway.

But I'm probably just nosy!
