I am planning to start my own mobile bussiness for Reflexology. Can any help me with a sample of a consultation form to be used.

What forms did you use with your 'case histories'? You can always modify them to suit yourself.

I used one that included TCM , diet about 6 pages but I went for a course in Swedish massage and the form they used only had the contra-indications on ang the declaration by the patient. I just think the forms we used during training will scare the clients away because they are so long to complete Does that make sense?

You could make one up, asking relevant questions about contra-indications, such as does the client have any of the following: High/low blood pressure; recent thrombosis; recent infection; heart condition.
I go through my consult form with my new clients, rather than letting them fill it in - this means that I get to know them a little bit more, and helps them to relax if they are nervous - there is also an excellent thread you could look at for ideas:
