Hi SerendipitySpa,
You will find that people have differing opinions on this topic. Some feel "the bigger the better" whereas others feel that "less is more". I've been trained in the "less is more" way and tend to use tumbles, points and reasonably-sized stones, especially when it comes to placing them upon a person's body. There are some stones which only form in small sizes; are comparatively harder to come by and hence more expensive to buy, so you are limited when using these and there are some small stones which can have quite a profound effect (e.g. phenakite). You go with what you feel is right for you and the person you are treating.
